Faculty, keep in mind: The due dates to submit Spring 2015 grades are May 15, 2015 for Graduating Students and May 21, 2015 for Non-graduating Students.
IMPORTANT: This form cannot be submitted electronically. Please type in the required information, print and sign the form and have it notarized.
Enclose with this application a $100.00 check payable to Columbia University, as well as photocopies of two (2) proofs of identification. Acceptable forms of identification are: passport, birth certificate, driver's license, social security card.
Replacement diplomas display the signatures of the current Columbia University President and Dean of your school but show the original date of the award.
